
Patients may be referred for Inpatient Admission to the Turning Point Rehabilitation Center by a physician, hospital social worker, discharge planner, other health care provider, family member, or the patient him/herself.
Our Admissions Coordinator and a nurse will complete forms for the pre-admission screening process. This information is shared with the physiatrist, who makes a decision on the appropriateness of treatment in Turning Point's inpatient program. The admissions coordinator will contact the referral source regarding the physiatrist's decision and will make arrangements for admission if appropriate.
(Physical Therapy, Occupational Therapy, Speech/Language Services):
The physician must write a prescription for the type of therapy and include the diagnosis and any other pertinent information. The patient can then contact our department to schedule an initial appointment.
OR
The physician's office may directly call or FAX in the Therapy referral. If the order is called in, please also send a written order. The Turning Point scheduler will then contact the patient to schedule an initial appointment.
